Your primary care physician is looking for disease. If your bloodwork lands inside the reference range, you’re fine, and the appointment ends. A longevity clinic is looking for trajectory. Same blood draw, different question: not “are you sick,” but “where is this heading, and can we change it?”

In practice, that means a much wider diagnostic net than an annual physical. Expect some combination of expanded bloodwork, hormone panels, inflammatory markers, body composition scanning, cardiorespiratory fitness testing, and biological age estimates. From there, clinics build protocols—hormone optimization, peptides, IV nutrients, supplementation, exercise prescriptions, sleep work, sometimes hyperbaric oxygen or regenerative therapies.

What it isn’t: a guarantee. The diagnostics here are largely real and well-established. A DEXA scan measures what it says it measures, and VO2 max is one of the better-validated predictors of all-cause mortality in medicine. The interventions are a more mixed bag. Some are FDA-approved and decades old. Others are promising, early, and sold with more confidence than the evidence currently supports. A good clinic will tell you which is which without being asked.

What it costs in Miami

Pricing splits into three tiers, and knowing which one you’re shopping in saves a lot of time.

Consultation-first. The lowest barrier to entry. Consultation fees vary by clinic, with a lot of them offering complimentary initial consultations. Program-based. You pay for a diagnostic workup and a protocol, then per treatment. Most Miami clinics operate this way. Costs vary enormously depending on what gets prescribed.

Membership. The high end. Where memberships are published, the top of the Miami market runs from $6,000 to $25,000 a year, typically bundling discounted services, physician access, and events. These make sense if you’re going to be in the chair constantly. They’re an expensive way to buy a few IV drips. Concierge practices like The Agatston Center and SM Concierge Medicine quote privately, and there the membership buys you a doctor rather than a menu.

Almost none of it is covered by insurance. Diagnostic labs sometimes are, if your physician can justify them as medically necessary. Assume the rest is out of pocket and you won’t be surprised.

Best for: Athletes and hard trainers who want recovery infrastructure, not a wellness membership.

Evolve draws its own line in the sand on the homepage: “We are not a spa.” The clinic is built for recovery and performance, and the equipment list backs it up. Hyperbaric oxygen, EBOO, shockwave therapy, PEMF, red light, lymphatic drainage, IV therapy, and regenerative medicine all sit under one roof on Washington Avenue in South Beach.

What separates it from the drip-lounge tier is the diagnostic layer underneath. Founder Connor Wade, an open self-experimenter who runs the clinic’s protocols on himself before recommending them, built Evolve around measurement. Best for: Anyone whose longevity plan starts with their heart.

If you’ve ever had a coronary calcium score, you’ve used Dr. Arthur Agatston’s work. The cardiologist created the Agatston score in the 1980s, and it’s still the standard way doctors measure calcified plaque in the arteries. Most people know him for something else, though: The South Beach Diet.

His concierge practice on Michigan Avenue is the most conventionally medical clinic on this list, and that’s the point. The philosophy fits in a sentence from the center itself: “the best time to treat heart disease is before it develops.” Diagnostics center on the heart, with CT calcium scoring, AI-enabled coronary CTA, and an AI-powered version of the calcium score the center calls Agatston 2.0. Metabolic testing and hormonal health assessments round out the picture.

The longevity program layers sleep, nutrition, fitness planning, and ongoing monitoring on top of full concierge primary care, with same-day or next-day appointments. The bench is deep for a boutique practice. Chief medical officer Dr. Judi Woolger and Drs. Founded in 2013 by Ross Bloom alongside his father, Dr. Martin G. Bloom, the biostation predates most of Miami’s longevity boom. That longevity, the institutional kind, is the pitch.

The approach is functional medicine and healthy aging rather than single-concern treatment. Programs span weight management, cognitive function, recovery, hormone balance, immune health, sexual wellness, and skin rejuvenation, with providers tailoring compounds and protocols to health history rather than running everyone through the same sequence.

Hall Longevity Clinic

Best for: People ready to pay for a physician relationship, not a treatment menu.

Dr. Michael J. Hall has been practicing in Miami Beach for more than two decades, which in this category makes him something close to an elder statesman. He’s board-certified in family medicine, did fellowship training in integrative and functional medicine at the University of Miami, and has a book, Infinite Youth: Solving the Aging Equation, on the way. His clinic on Meridian Avenue is about as direct a fit for the word “longevity” as Miami has.

The workup is thorough. Expanded Quest bloodwork covers cardiometabolic, inflammatory, hormonal, and nutrient markers, and there’s a full hormone panel, GI-MAP stool testing, toxic-metals screening, a body composition and performance assessment, and the Galleri multi-cancer early detection blood test for eligible patients. Hall organizes all of it around his own framework, the “Aging Funnel,” before anything gets prescribed.

The treatment side reaches further out: hormone therapy when it’s clinically appropriate, peptides, and regenerative medicine that includes exosomes and other therapies still well ahead of the evidence. To the clinic’s credit, its own site says so, with evidence caveats and links to FDA warnings sitting next to the service descriptions. That’s rarer than it should be, and it’s exactly the transparency we tell readers to look for.

This is the most expensive consultation on the list, and Hall publishes it rather than making you ask. An in-office consult starts at $750 and telemedicine at $500, plus a $100 non-refundable booking fee. The clinic doesn’t accept insurance, Medicare included. Longer-term patients move into the Hall Continuum, a concierge membership whose terms are set privately. The tagline sums up the pitch: “Longevity is not a visit. It is a relationship.”

Best for: Patients who want a board-certified internist running the whole plan.

Dr. Yeisel Barquin is a board-certified internal medicine physician, and Integrative Health Miami reads like a practice built by one. Every patient starts with a comprehensive medical consultation—history, goals, underlying concerns—before anything gets prescribed.

The emphasis here is explicitly not on chasing the newest compound. Depending on need, therapy might support tissue recovery, hormone balance, athletic performance, chronic pain, or general vitality, delivered by injection, nasal spray, or topical cream. Protocols get monitored and adjusted as goals shift, with everything positioned as one component of preventive care rather than the point of it.

Dr. Nina Gupta has spent more than 37 years in medicine, starting with internal medicine and then pulmonary and critical care training at SUNY in Brooklyn. She’s a fellow in anti-aging and regenerative medicine through the A4M, and her practice off West Dixie Highway is the only one on this list in Aventura. That’s a real advantage if Aventura is home.

Longevity work starts with a healthspan evaluation that tracks the markers preventive cardiologists care about, including ApoB, Lp(a), hs-CRP, fasting insulin, and A1c, with continuous glucose monitoring available. Hormone panels, nutrient levels, body composition, bone density, and biological age testing fill in the rest. From there, protocols can include bioidentical hormone therapy for women and men, peptides, NAD+ and IV nutrition, and brain health and sleep support. SM Concierge Medicine is the clearest illustration on this list of ongoing care versus one-off treatments. Most longevity clinics sell you a workup and a protocol. This one sells you a doctor. Dr. Samuel Maghidman, trained in internal medicine and infectious disease at the University of Texas at Houston, has run a concierge practice in Miami Beach for more than 15 years, with admitting privileges at Mount Sinai Medical Center and Mercy Hospital.

How to vet a longevity clinic

Five questions that separate a medical practice from a membership business.

Who is actually prescribing? Ask for the name and credentials of the physician or nurse practitioner overseeing your protocol, and ask whether you’ll see that person again. A clinic that can’t answer this quickly is telling you something.

What gets measured before anything gets prescribed? A meaningful baseline should exist before you start. If a clinic is ready to sell you a protocol before it has seen any labs, that’s a retail transaction wearing a white coat.

What’s the follow-up schedule? Repeat testing during active treatment is how anyone knows whether it’s working. Ask for the interval in writing.

What’s the total cost? Consultation, labs, medication, supplies, and follow-ups. Get the full number before you commit, not one surprise charge at a time.

Will they tell you what’s experimental? This is the real test. Some of what’s on offer is decades old and well studied, while some is newer. A clinician who distinguishes between the two without being pushed is one worth keeping.
